On-Line Incident Report Form

Note: Filing a False report is a violation of society rules punishable by expulsion from the SCA.

Fill out this report for anyone who behaved in such a manor that action either was (or in your opinion) should be taken by this office or any office of the SCA or Kingdom of AEthelmearc. 

If you mean to file an INJURY REPORT then this is the incorrect form.

If multiple offenders are involved then fill out separate reports for each one.
